My toe nail was bruised in April,but it didn't swell up until a month ago.Why is that and what can I do?

The nail has lifted off the nail bed,and it's very uncomfortable for me to walk because there's so much pressure built in there. How long would it take to heal? I've already seen a podiatrist, and he said it would take a month. It's been more than a month and it's gotten better and now worse.My toe nail was bruised in April,but it didn't swell up until a month ago.Why is that and what can I do?
